In 2024, CMS elevated the importance of functional outcomes by adopting the Discharge Function Score across the HH Expanded VBP Model, SNF QRP, and SNF VBP, directly tying payment to patient progress. This session gives PTs and OTs a clear, practical overview of the measure, how it is calculated, and why it matters. Attendees will learn actionable strategies for integrating the score into daily practice, strengthening documentation, and ensuring that each patient's functional gains are accurately captured. Participants will leave prepared to collaborate with their agencies and facilities to improve quality and performance, and to highlight the essential value of rehabilitation in post-acute care.
