As a clinical reviewer working with SNF documentation, I recognized all of the examples of unvalidated documentation as those I see every day. I wonder often if the therapists treating are actual therapists because the documentation is so poor. I would recommend this course for all SNF therapists.

It validated my insistence on quality charting for which, sadly I'm ALWAYS on the hot seat because the performance standard seems to be based on PRODUCTIVITY not QUALITY nor OUTCOMES. I hate this. I want to chart narratively like when I started PT (S.O.A.P. notes on carbon paper! lol) but my ADD can't seem to swim through all the pages and pages of drop-down menus. well presented. Thanks for your vast amounts of experience. The actual examples were helpful too.

I do tend to argue with people asking to write more specific note as they tend to get us in more trouble than help us. We were always told to be vague and general and use less therapy words because reviewers couldn’t understand our notes and now we are being told to be more specific and use professional language. It’s frustrating and exhausting and feels like we shouldn’t be doing therapy like walking or HEP if they aren’t skilled.

I would have preferred to hear more about the EMRs specifically and how to work within the confines of the EMR to be able to meet the requirements -- or how the EMR helps to collect data and metrics for reporting purposes and also meets documentation requirements.

Very good, specific real life examples. Helpful to know what auditors are looking for. I have been confused about some of the buildable sentences on my software, thinking they are there to help me, and to include information needed to avoid auditing. It has made it harder to make my documentation specific. This course has validated that my specific documentation is still needed and I can skip some of the auto building that is nonspecific.

I'm pretty sure all therapists know if their documentation is sufficient, it's just easy to get "lazy" with documentation due to time restraints. It's always good to have a review to remind you to keep your documentation up to par. But why did he keep driving home that patient's need to be improving? Have they done away with maintenance therapy?
