Course Abstract

This course addresses updated concepts of mechanical treatment for patients with unspecific low back pain. Two classification systems are widely used for the treatment of patients with lumbar impairments (i.e. Treatment Based Classification [TBC] and Mechanical Diagnosis and Treatment [MDT]). TBC guides physical therapists in daily practice through identification of clinical patterns called clinical prediction rules. The use of clinical prediction rules constitutes a controversial topic in our field. While some clinicians consider this approach to be “too simplistic", others advocate its use based on promising emerging evidence. On the other hand, MDT, uses, among others, two common examination findings to classify patients in subgroups (i.e. directional preference and centralization). Both systems have interpreted these clinical findings inconsistently leading to variable results in terms of prevalence and predicted validity. Even though these two approaches appear to be very different, there are interesting commonalities that can be used in clinical practice. The intent of this presentation is far from advocating one system over the other; instead, it is an invitation to consider integrating validated constructs used in both systems in clinical decision-making. This is an advanced level course, recommend background knowledge for understanding of this update.
